Requesting Your Records

At Psychotherapy by Dr. Azzam, PLLC, we believe in your right to access your health information. If you would like a copy of your medical records:

  • Please email your request to cazzamphd@gmail.com.

  • We will provide your electronic records within 15 business days.

  • You may also request a Medical Records Request Form in session.
